HEALTH ASSESSMENT AND IMMUNIZATION REQUIREMENTS Within thirty (30) calendar days of their first enrollment date, all pupils must show evidence of immunization against diseases as outlined in state law. Such evidence of immunizations must be provided in the form of an official document from a licensed health care provider or health department clinician. Additionally, all students entering a North Carolina Public School for the first time must present a completed Health Assessment Form within 30 calendar days from their first date of attendance. Health Assessments communicate health concerns and information from parents and physicians for all children entering a North Carolina Public School for the first time. This allows issues that affect learning to be addressed. State law requires that every child entering a North Carolina Public School receive a health assessment. This requirement has been put in place since the 2016-17 school year. Parents/guardians must submit a completed health assessment for each child who is seeking admission into a North Carolina Public School for the first time. The health assessment must have been completed no more than 12 months prior to the date the child would've first been eligible for entry into public school (G.S. 130A-440).
